New Patients

New patients are asked to complete the Patient Registration form and submit it with your insurance card, if applicable, at the time of sign-in for your initial evaluation/first appointment.

Please read the HIPPA Privacy Act Information form carefully as it pertains to your rights and privacy as a patient at Foothills Sports Medicine. This form must be signed and turned in with your patient registration and cancel/no show policy forms at the time of your initial evaluation/first appointment.

Please fill out and sign the Consent for Care form granting consent for care to Foothills Sports Medicine.

Cancellation/No Show Policy:  This is our policy on cancellations and no shows. New patients are required to read and sign the Cancellation/No Show Policy form prior to therapy.

Please note that these forms are meant to give you a head start when coming for your first visit. There may be other forms needed to fill out based on insurance and financial consent information.
